Responsibilities Responsible for coding all inpatient and/or outpatient hospital records, ED records, Home Health & Hospice records, and/or professional fee services for PMG specialty providers. Use ICD-10 CM, CPT-4 classifications. Ensure adherence to Hospital and Departmental Policies and Procedures. Review patient medical records, assigning appropriate CPT, ICD-10, and MS-DRG codes for financial reimbursement and research in accordance with Federal Regulations and Hospital and Departmental policies. Research medical records via multiple systems to complete coding promptly. Maintain accounts receivable by identifying lost documents and ensuring all encounters coded, generating queries as needed. Maintain up-to-date technical knowledge of legal and regulatory information (ICD, CPT, HCPCS, DRG, APC, HHRG updates). Resolve pre-bill edits, denials, etc. for assigned accounts. Qualifications High school diploma/GED required. Must have one of the following coding certifications at time of hire: CCS, CCS‑P, CPC‑H, or RHIT/RHIA; and achieve one of these credentials within one year of hire. Three to five years experience as a coder required.
