Job Description

1. Review medical record documentation to identify all services provided within the hospital

and by physicians.

2. Assigns appropriate CPT and ICD-9/10 diagnosis and procedure codes to accurately support the

need for each service. Reviews record for services provided, but not adequately

documented in the medical record. Advises supervisor, department and/or physician of

deficiencies to support the charge capture of all billing services.

3. Analyzes and resolves claim rejects and denials from the billing office related to

coding issues.

4. Assists with billing and documentation training in daily interactions with departments

and physicians and other routine training sessions.

5. Identifies trends/problems in medical documentation and recommends possible solutions.

6. Will be required to use hands, arms, sit long periods, bend, stoop, climb, and move your

whole body.

7. Will be required to have good people skills as we deal with the public as well as other

employees. Good communication skills are a must in writing, by telephone, e-mail or with people.

8. Employee may be directed to perform other duties as requested by supervisor.

*High School Diploma

*Coding Certification or successfully completed Coding & Medical Terminology Classes.

*Employee should have a working knowledge of CPT and ICD-9/10 and modifier use.

*Knowledge and/or experience in health care setting are a plus.

*Familiarity of personal computer operations.

*Basic knowledge of word processing
