HCC Coder - Abacus Health Job Summary Performs pre-visit patient chart review to find Hierarchical Condition Category and Risk Adjustment Factor (HCC/RAF) coding opportunities for multiple Primary Care providers using expertise in ICD-10 coding practices, Medicare Guidelines and Value Based principles. Responsibilities Performs chart reviews as required. Demonstrates high level of organizational skills in prioritizing tasks and managing workload. Maintains current coding materials (CPT, ICD-10, HCPCS), and Medicare guidelines, with a focus on HCC/RAF and other Value Based principles. Disseminates new information and policy changes to staff and physicians. Ensures integrity, confidentiality, and security of records. Minimum Qualifications Education: Certified Professional Coder (CPC) certification is required. The coder is expected to enroll in continuing education courses to maintain certification. Work Experience: Two years of coding experience using ICD-10. Three years of experience within primary care relating to physician practice coding and chart documentation review. Preferred Experience: HCC/RAF experience preferred.
