HIM Coder Analyst II

ROLE IMPACT
Support accurate reimbursement, regulatory compliance, and quality reporting by reviewing outpatient medical records and assigning accurate ICD-10-CM and CPT-4 codes. Success in this role is measured by coding accuracy, productivity, effective provider communication, and adherence to coding guidelines and documentation standards.
KEY RESPONSIBILITIES
Review and interpret outpatient medical records to assign ICD-10-CM and CPT-4 codes with the highest level of specificity for ambulatory surgery, observation, special procedures, emergency department, ancillary, and clinic encounters.
Code primarily complex ambulatory surgery and observation cases while maintaining a minimum coding accuracy rate of 95%.
Abstract required clinical and demographic information into the electronic health record to support billing and reporting.
Submit physician queries to clarify documentation and collaborate with clinical documentation specialists to ensure accurate coding.
Identify documentation concerns, report risk management issues, and remain current on coding regulations, payer guidelines, and industry updates.
LICENSES & CERTIFICATIONS
Current RHIA, RHIT, CCS, or CPC certification required.
Minimum of 2 years of current full-time hospital outpatient coding experience involving ICD-10-CM, CPT-4, ambulatory surgery, observation cases, and medical record abstracting.
Current AHIMA continuing education certification records required, when applicable.
