Hickory Hill Leisure Living consist of 3 Behavior Focused/Intensive Behavior Residential Habilitation Group Homes.

Responsibilities
Coordinate and monitor all medical services for residents.
Review and implement physician orders and treatment plans.
Schedule and track medical appointments, follow-ups, and specialist care.
Monitor chronic health conditions and ensure continuity of care.
Serve as primary liaison between the group home and external healthcare providers.
Participate in interdisciplinary team meetings and Individual Service Plan (ISP) development.
Conduct health-related risk assessments.
Respond to medical emergencies and provide clinical guidance as needed.
Oversee medication administration systems and ensure accuracy and compliance.
Qualifications
A Strong Willingness to Serve Others
Minimum of 2–3 years of relevant experience; experience working with individuals with intellectual/developmental disabilities preferred.
minimum requirement of an associate's degree with proof of completion
Valid Florida Driver’s License with an insurable driving record
Physical condition allowing for safe, repeated, satisfactory completion of essential functions of position defined in this description.
Able to pass in first 30 days of employment, background screening which includes FDLE, FBI fingerprint and local law check.
Core Competency, Zero Tolerance, CPR, First Aid, Behavior Assistant, TACT, AIDS/HIV, Orientation and Medication Certification training must be completed within the first 30 days of employment and kept current with required re-certifications.
Ability to pass both a pre-employment drug screen and random drug screening.
