Avocare Health Services provides tech-enabled advanced services and supports that allow patients to remain independent through effective self-management and a continual connection to their providers, resources and loved ones in the communities where they want to live. From Skilled Home Care delivered in the home to Virtual Care delivered through our call center, we actively engage patients to keep them informed and assess changes in their health or living situation. We leave no patient behind and can keep them connected throughout their life. We believe all patients should be able to age in place in their own home with dignity, independence and freedom.
