Community Health Systems, Inc. is a Federally Qualified Health Center in Beloit, WI.  CHS currently offer 4 service lines, Medical, Dental, Behavioral Health, and Chiropractic Care.  We serve over 8,000 patients annually.  We accept Medicare, Medicaid, Commercial Insurances, and Self-Pay.  We currently have 112 employees.
