Follow appropriate guidelines and policies to code accurately from physician documentation within the medical record. Queries physicians for diagnoses or missing/ambiguous information for accurate coding. Apply organizational documentation policies and procedures in conjunction with official coding guidelines. Applies knowledge of coding and DRG assignment for ethical and optimal reimbursement.
