Central Florida Health Care was founded in 1972 after a local citrus processing firm recognized the importance of adequate healthcare, social services, and childcare for farm workers in Frostproof, Florida. The first clinic opened with just one doctor and a nurse and began serving the farm workers in the Frostproof community.

Our mission to serve as the premier Patient-Centered Medical Home offering quality, affordable primary and preventive healthcare has empowered us to continue expanding our services. CFHC now has 17 sites in Polk, Highlands, and Hardee Counties, including one unique Community Partnership School located at Crystal Lake Elementary School in Lakeland. CFHC also has three mobile healthcare units to serve the most rural areas of the tri-county site.

Services include adult and pediatric medical and dental care, OB/GYN, behavioral health, infectious disease, lab & x-ray, and nutritional counseling. CFHC also provides specialty services including dermatology, endocrinology, optometry, podiatry, and rheumatology. CFHC remains dedicated to identifying opportunities that will support current endeavors and assist in expanding services for the community.
