1. Review medical record documentation to identify all services provided within the hospital and by physicians. 2. Assigns appropriate CPT and ICD-9/10 diagnosis and procedure codes to accurately support the need for each service. Reviews record for services provided, but not adequately documented in the medical record. Advises supervisor, department and/or physician of deficiencies to support the charge capture of all billing services. 3.
