The Coder is responsible for timely review of patient records in order to identify an appropriate selection of codes which will accurately reflect the reason for admission, extent of care received, and level of severity of illness. Coder is further responsible for insuring that all data elements required for federal and state reporting are collected and included in the patient's demographic record. Abstracts records into Hospital's computer system. This job description in no way states or implies that these are the only duties to be performed by this employee. Assigns appropriate ICD-10-CM, ICD-10-PCS and CPT codes to medical records. Reviews entire record for all diagnosis and surgical procedures.
