Job Responsibilities: Review, code, data entry and interpret with accuracy and complete patient data for medical office, outpatient, inpatient, handwritten chart entries, practitioner orders and other related documentation to ensure accurate information is being submitted for billing. Obtain accurate and complete patient data through the review of the medical record, discharge summary, history and physical, consultation, progress notes, and laboratory, radiology, operative and pathology reports. Maintains competence in and up-to-date knowledge of healthcare compliance requirements, practices, trends, coding rules and standards in areas of responsibility.
