This is an exciting opportunity to join and oversee a team committed to delivering exceptional, patient-centered care in a clinic setting and in the homes of patients. The Comprehensive Care Center provides clinic and Home-bound services for adult (18 years +) polychronic patients, who require longer appointment times and a care team model approach.
