Job Title Help Job Description Reviews medical records for completeness, including required identifiers, signatures, dates, and reports associated with services rendered. Evaluates documentation for accuracy, consistency, medical necessity, and appropriate modifier usage; verifies that final diagnoses accurately reflect care provided. Reviews provider documentation to ensure appropriate Evaluation and Management (E/M) levels and correct CPT code assignment. Assigns and sequences ICD, CPT, HCPCS, CDT, and DSM codes based on documented diagnoses and procedures. Ensures diagnostic and procedural terminology aligns with current medical nomenclature and official coding guidelines.

Responsible for claims billing and Follow up on claim denials and outstanding AR. Minimum Qualifications - For full consideration, applicants are encouraged to upload license and/or certification if required of the position High School diploma/GED or equivalent work experience Follow-up payer billing experience. History of successful payor denial appeals. Experience with government and third-party payors. Including current regulations, time limitations and procedures. Extensive knowledge of Medicare and Medicare Risk Plans is required. Knowledge of Medicare, Medicare Risk, Tricare and VA Plans is required. Extensive knowledge of medical billing/follow-up/reimbursement requirements and procedures for government programs, private third party insurance and private individuals.
